Um, it’s only unhygienic if you don’t actually wash it regularly. It’s not a condition that needs to be “cured”.
Not being able to retract it has nothing do with being unhygienic - poor self-care is what is what is unhygienic. Just use a shower head and mild soap if necessary and you’re all good.

Maybe it’s fine in your case. For some people it’s so tight that their pee shoots out sideways, any amount of self care is not going to be able to clean sufficiently under there.
If pee can shoot out of it, that means water can get in as well. Use a shower head with a decent pressure, stretch your foreskin back as much as you can comfortably do it, angle the shower head towards the inside, moving it in a circular manner and you’re done. Oh, and use a TP to wipe your tip after peeing.
